2. Complete the HSA application form
3. Make a check payable to First Bank
& Trust for the Setup Fee of $25.00 and your initial deposit
($50.00 minimum)
4. Attach photocopy of valid driver’s
license to application
You can expect your welcome packet and Visa Debit Card in approximately
